On 30 June 2005, President of Ukraine Viktor Yushchenko signed the Decree on the Establishment of Visa-Free Regime for Citizens of the United States of America, which introduced visa-free regime of entry to Ukraine and transit through its territory for U.S. citizens who may stay in Ukraine up to 90 days within the period of six months.
Attention: This regulation is not extended to the immigrant visas, student visas with the purpose to study in Ukraine and for those U.S. citizens, who enter Ukraine with the purpose to work at the U.S. diplomatic/consular missions in Ukraine and
to the cases when the U.S. citizens stay in Ukraine more than 90 days within the period of six months.
Starting August 1, 2005 Ukraine sets visa free regime for citizens of
Japan and Canada.
On July 19, 2005 and July 26, 2005 President of Ukraine Victor Yushchenko signed Decrees #1107/2005 and #1134/2005, which introduced visa-free regime of entry to Ukraine and transit through its territory for citizens of Japan and Canada who may stay in Ukraine without visas up to 90 days. A passport valid for six months beyond the planned date of travel is required.
For the Japan and Canadian citizens who enter Ukraine with the purpose of employment, permanent residency, study and work at the diplomatic missions of the above mentioned countries in Ukraine or with any other purpose if they are going to stay in Ukraine for more than 90 days, visas are required.
On July 26, 2005 President of Ukraine Victor Yushchenko signed the Decree #1131/2005, that introduced
visa-free regime of entry to Ukraine and transit through it territory for
citizens of European Union, Switzerland and Liechtenstein who may stay in Ukraine without visas up to 90 days. A passport valid for six months beyond the planned date of travel is required.
For citizens of the above mentioned countries who enter Ukraine with the purpose of employment, permanent residency, study and work at the diplomatic missions of the above mentioned countries in Ukraine or with any other purpose if they are going to stay in Ukraine for more than 90 days,
visas are required.
FOR ALL VISAS THE FOLLOWING MUST BE SUBMITTED:
- Original (no copies!) passport or travel
document, both hereinafter referred to as "the travel document",
valid for at least one month after the expiration date of the requested
visa and having two blank visa designated pages. Please note that
the so-called Green Card (i.e., Resident Alien Card) is not
valid for entry into Ukraine;

- One Visa Application form.

- Two photographes - recent, passport-sized, resembling applicant's
travel document photo.

Together with all items listed
above the following documents, each hereinafter referred to as
"the
supporting document", related to the purpose
of entry, must be submitted in support of the Application for:

Long-term one-entry
TRANSIT
VISA (VB)

an original invitation (in some exclusive
cases a legible fax version may be also accepted) from a legal
entity duly registered in Ukraine, typed on its letterhead containing
its official address and telephone number, signed by an authorized
officer (other than the applicant) of the entity, bearing
its seal and accompanied, when applicable, by a clear copy of its
valid Certificate of Registration in Ukraine. The invitation must
detail the purpose and if necessary the number of visits, exact
travel dates, cities and/or towns to be visited and provide basic
passport data of the invitee and be duly arranged by the internal
affairs agencies.
SHORT-TERM VISA

an original letter of invitation from a Ukrainian religious
organization for a foreign missionary must contain or be accompanied
by a written confirmation under the signature and seal of a responsible
official of the State Committee of Ukraine on Religious Matters
or of the department on religious matters of a regional or city
administration depending on where the sponsoring organization is
registered (according to the acting Law of Ukraine foreign citizens
staying temporarily in Ukraine can perform religious activities
only in those religious organizations which have invited them and
provided official approval by state authorities by which the statute
of the inviting organization was registered).
LONG-TERM ONE-ENTRY VISA

for individuals providing Humanitarian Aid, a visa is
issued only on the basis of a letter of support from the
Commission at the Cabinet of Ministers of Ukraine for coordination,
acceptance, transportation, security and distribution of humanitarian
aid rendered by foreign States.
IMPORTANT: the Consular
District (the area assigned to a Consular Post for the exercise
of consular functions - Article 1 of the Vienna Convention on Consular
Relations and Optional Protocols, 04/24/1963) of the Consulate General
of Ukraine in Chicago includes States of Illinois, Indiana, Iowa, Kansas,
Michigan, Minnesota, Nebraska, North Dakota, South Dakota, Wisconsin.

Office.

AND SUBMIT ALL THE REQUIRED DOCUMENTS.

ALL APPLICATION PAPERS MUST BE CORRECTLY AND COMPLETELY FILLED OUT
(as well as a proper payment attached). FAILURE TO COMPLY OR
PROVIDING INCORRECT OR MISLEADING INFORMATION WILL RESULT IN THE RETURN
OF THE APPLICATION UNPROCESSED.
