How, When and Where to Apply


Please note that beginning on September 15, 1999, the Consulate General of Ukraine in Chicago accepts applications filled out only on the new revised application forms. You can also print it from our website. Information on the form must be typed or hand printed.


IMPORTANT: the Consular District (the area assigned to a Consular Post for the exercise of consular functions - Article 1 of the Vienna Convention on Consular Relations and Optional Protocols, 04/24/1963) of the Consulate General of Ukraine in Chicago includes States of Illinois, Indiana, Iowa, Kansas, Michigan, Minnesota, Nebraska, North Dakota, South Dakota, Wisconsin.

Please be sure to send your Visa Application Form to the appropriate Consular Office.

The documents are expected to be submitted to the Consulate no sooner than three months prior to the date of the entry into Ukraine.

In case of need, a Consular Officer may request additional documents which would in a better way specify the purpose of the alien's trip or invite a certain person for an additional interview.


When an application is submitted in person or through a travel agency or visa service, a numbered receipt (a pick-up slip) is issued to the submitter. This receipt should be presented at the Visa Office of the Consulate General when the visa is claimed. The travel document will be released to the submitter of the receipt unless its owner or other submitter has provided other written notarized instructions at the time of the application. If the receipt is lost the travel document will be released only to its holder or at the written notarized request of the holder to his/her appointee.

If you apply by mail, you should enclose a self-addressed and stamped return envelope or prepaid airbill for USA delivery only, bearing your account number and showing your company as both the shipper and the recipient. We do not accept airbills marked "Bill Recipient", or "Bill Third Party", or "Bill Credit Card", or C.O.D.

For applicants' convenience, documents may be submitted to the Consulate General by mail ( Express Mail, Priority Mail only), messenger or courier services.

If you fail to follow the above-mentioned requirements, your visa application will not be processed.

If the travel document is to be sent back, a prepaid, self-addressed return envelope or a courier service's air bill is required from the submitter. When there is no enclosed prepaid, self-addressed return envelope or a courier service's air bill, the travel document can upon its owner or other submitter notarized request, be kept at the Consulate General without charge for personal pickup or may be mailed to a designated address by regular mail.

If no instruction has been given, the travel document not submitted in person will be held at the Consulate General also gratis until instructions are forthcoming.

If the papers are not acceptable for any reason or some documents are missing, we will return them by regular mail.


When forwarding documents to us, please specify "Attention: Visa Section".

We do not accept documents for international delivery.

The Consulate General is not responsible for lost, damaged, misdirected, or late-delivered mail.


PLEASE NOTE:

  1. Visa processing starts only after we have registered your application and collected all necessary documents.

  2. A drop-off day is not counted as a business day.

  3. If you apply by mail please allow one more day processing time.

  4. Any application received on Wednesdays, weekends, official Ukrainian and US holidays will be registered by the Consulate only on the next business day.

ALL APPLICANTS SHOULD CAREFULLY READ THE VISA REQUIREMENTS AND SUBMIT ALL THE REQUIRED DOCUMENTS.

ALL APPLICATION PAPERS MUST BE CORRECTLY AND COMPLETELY FILLED OUT (as well as a proper payment attached). FAILURE TO COMPLY OR PROVIDING INCORRECT OR MISLEADING INFORMATION WILL RESULT IN THE RETURN OF THE APPLICATION UNPROCESSED.

BEFORE PURCHASING AIRLINE TICKETS.
